Accomplishments represent actions taken that are recognised by the System. Each Accomplishment has a set of requirements specified in their description that must be met and an associated reward. They are also known as 'Achievements'.
In addition to the specified requirements to achieve each accomplishment, there are a few unstated ones that apply to all of them.
Accomplishments are given ranks using the standard rarity scheme: Copper, Iron, Silver, Gold, Platinum, Palladium, Crysium, Exium, Galactium, Unique. So far, only Silver-ranked Accomplishments have provided information on how to achieve the next higher rank for that Accomplishment in text. In addition, no ranks other than Silver, Gold, Platinum and Unique have been presented, making the others currently hypothetical.
Rewards are relatively consistent for each Accomplishment, varying only in the nature of the Ability gained. Awards appear to be the same regardless of who earns it, though unspecified details (like the exact Ability) do vary.
The System allows the recipient to reject the awarded Ability, in which case it is replaced by a Voucher to choose an Ability of the same broad type (Active or Passive) at any lower rarity. For instance, if an offered Active Silver Ability is rejected, the recipient gets a Voucher for any Active Iron-rarity or lower Ability.
Some Accomplishments give nonstandard awards. These are called out in their descriptions. Otherwise, only the type of Ability is specified and the Credits and Merit are assumed.
The UHF database lists 96,564 known Achievements at Major Quinn's clearance, (Arc 1 - Chapter 23 - Achievements) and 112,761 at Captain Cross's clearance (Arc 1 - Intermission 1 - Emergency Meeting). We know far fewer than that.
A note on Accomplishment names. The first four Accomplishments revealed in the story were of the form an adjective followed by a standard rarity in parentheses, then a name (e.g. Senior (Silver) Marksman). All later Accomplishments do not follow this pattern. This extra text at the start of the Accomplishment name has been left out in the descriptions below.
Have at least 100 Integrated enemies killed by your weapons, without you being the wielder.
Nonstandard reward: 3 Skill Vouchers of Silver rarity or below.
Have at least 1000 Integrated enemies killed by your weapons, without you being the wielder.
Successfully infiltrate and complete a Faction Mission deep behind the enemy lines three times in one Battlefield.
Ability reward: Stealth or Assassin tag
Successfully infiltrate and complete a Faction Mission deep behind the enemy lines five times in one Battlefield.
Successfully lead a squad as a scout and detect traps, ambushes and dangers that would lead to failure of a mission three times in one Battlefield.
Ability reward: Perception based
Successfully lead a squad as a scout and detect traps, ambushes and dangers that would lead to failure of a mission five times in one Battlefield.
Successfully kill more than 95% of all enemies hit in one shot.
Ability reward: Dexterity or Perception based
Successfully kill more than 97% of all enemies hit in one shot.
Successfully destroy two mission-critical objective targets in one Battlefield.
Ability reward: Perception based
Successfully destroy three mission-critical objective targets in one Battlefield.
Successfully hit more than 95% of all shots taken.
Ability reward: Dexterity or Perception based
Successfully hit more than 97% of all shots taken.

Defeat two or more Psykers in direct confrontation, without the use of any Active Psychic Powers or Psychic-related Abilities.

Reach Level 10 before the end of your 2nd month of Integration.

Successfully kill more than 50 soldiers of the Stellar Republic that are actively using their Faction Trait.
Ability reward: A Technology Tier-Up Equipment Voucher replaces the Ability.
Successfully kill more than 75 soldiers of the Stellar Republic that are actively using their Faction Trait.
Successfully neutralise over 20 enemies at least one Tier higher than yourself within 15 minutes, without being detected and without using area-of-effect Abilities or weapons.
Ability reward: Stealth or Assassin tag
Stay the highest PV member of your Faction at your current Tier and Level for at least one month.
Nonstandard reward: Gold-rarity Ability Choice Voucher
